Orthokeratology, commonly known as Ortho-K, is a non-surgical vision correction method that allows individuals with myopia (nearsightedness) to enjoy clear vision throughout the day—without the use of glasses or contact lenses. By gently reshaping the cornea overnight using specially designed lenses, Ortho-K provides an effective and reversible alternative to traditional corrective methods. It has also shown promise in slowing the progression of myopia in children, making it a valuable tool for long-term eye health.
Ortho-K treatment involves wearing custom-designed, gas-permeable contact lenses while you sleep. These lenses temporarily reshape the cornea—the transparent front part of the eye responsible for focusing light. In individuals with myopia, the eye’s shape causes light to focus in front of the retina rather than directly on it, resulting in blurred distance vision. Ortho-K lenses gently flatten the central cornea overnight, allowing light to focus more accurately on the retina by morning.
When the lenses are removed upon waking, most patients experience clear, unaided vision during the day. However, to maintain the effect, the lenses must be worn regularly as prescribed by your eye care provider.
Ortho-K works through a process known as corneal reshaping or corneal refractive therapy. The lenses exert mild pressure on the corneal surface, redistributing epithelial cells to alter the cornea’s curvature. This adjustment is temporary but effective in correcting refractive errors like myopia (and, in some cases, mild astigmatism and hyperopia).
It’s important to note that the reshaping effect does not permanently alter the cornea. Discontinuing use of the lenses will result in the eye gradually returning to its original shape and vision reverting to its prior state.
One of the most compelling benefits of Ortho-K is its potential to slow the progression of myopia in children. Unlike glasses or soft contact lenses that correct vision without affecting eye growth, Ortho-K alters peripheral retinal focus in a way that may signal the eye to slow down its elongation—a key factor in myopia progression.
Numerous clinical studies have shown that Ortho-K can reduce the rate of myopia progression by up to 40-60% in children. This makes it an excellent option for pediatric patients who are at risk of developing high myopia, which is associated with a higher risk of retinal detachment, glaucoma, and other vision-threatening conditions later in life.
Ortho-K is FDA-approved for both adults and children and is generally considered safe when prescribed and monitored by a qualified optometrist. It is particularly appealing for individuals who are not candidates for refractive surgery or those who participate in sports or activities where glasses or daytime contact lenses may be inconvenient.
However, not everyone is a suitable candidate for Ortho-K. A comprehensive eye exam and corneal topography are essential to determine eligibility. Your optometrist will evaluate your refractive error, corneal health, and lifestyle needs before recommending this treatment.
Proper hygiene and lens care are critical to ensuring the safety and effectiveness of Ortho-K treatment. Patients must follow specific guidelines for cleaning, storing, and handling their lenses to minimize the risk of eye infections such as microbial keratitis.
